Late Policy

Your appointment time has been reserved for your treatment; please arrive on time to get the full value out of your consultation.  If you know you are going to be late, please call and let us know so that we can be prepared for your arrival.  If you do not give notice, we will wait 10 minutes beyond the start time of your appointment.  If you have not arrived by then, your appointment will be cancelled.   

Cancellation Policy

In the event that you need to cancel an appointment, please give us the courtesy of as much notice as you can, but at least 24 hours.  We strive to provide the highest level of service.  Failure to cancel with sufficient notice denies an opportunity for another patient to be seen at the time reserved for you.

Payment

Payment in full is expected at the time of treatment.  Herb prices are non-negotiable.  Price of herbs fluctuate slightly with the market and cannot be guaranteed from week to week.  Your health is important to us and we hope to work with you to ensure that you are able to complete a full course of treatments and achieve the best results.  For this reason, we offer our herbal consultations on a sliding scale.  Please pay what you can afford within the recommended guidelines:

            Initial consultation:                      $35-50

            Follow-up consultations:             $15-35

We do not accept health insurance or file health insurance claims.  We accept payment by cash, check, Visa, MasterCard, and Discover.  Returned checks are subject to a $20 service charge.

Phone Calls and E-mails

Please call us or e-mail us when necessary!  We will respond as soon as possible, or within 24 hours Monday-Saturday.  Except for emergencies, phone and e-mail contacts are limited to 10 minutes.  We suggest a consultation appointment be made for all contacts that require beyond 10 minutes of our time.

Confidentiality and Privacy Practices

As health care providers, we are required by law to maintain and protect the confidentiality of your health information.  You must give us written consent to waive this confidentiality.  Exceptions to this rule are strictly limited to defined situations that include emergency care, quality assurance activities, public health, research, law enforcement activities, obtaining payment from third-party payers, and in consultation with other healthcare professionals.  Any other disclosures for the purpose of treatment, payment, or practice operations will be made only after obtaining your permission.

Ownership Policy

All orders placed through the Urban Herbs practitioner portal are the property of Urban Herbs and may be reproduced, shared, and sold as we see fit.
